Heli-coil fail

I had to redo 3 manifold bolt holes. I made a drilling jig to center a bit over the bad holes then I drilled the holes out to allow a 9/16" fine thread tap to make new threads. I then took a 9/16" fine thread G8 bolt put it in the lathe and drilled and tapped it for a 3/8" fine thread. I squared off the end. Tgen using a bolt threaded into it with a perfectly flat washer I threaded the insert in place with a bunch of Red Loctite making sure it was perfectly flush to the block. A lot of work but the results are stronger then stock threads.
 
Helicoil fail? Not a surprise at all. You are screwing with headbolt threads. The right drilling, and the correct heat will easy out them out
